From U8 to U18, the attached Roster/Game sheet MUST be provided to the Referee BEFORE the start of play. Save this form to your computer. The form is fillable, you can complete your team name, your name and your roster just once (as long as jersey numbers don't change) you may then save it and simply add in the opposing team name, date and location before heading out for each game. Claims for Injuries sustained during a sanctioned activity are to be submitted through the

EODSA Exhibition Game Process (Within District) - Process and Game Sheet
Teams must inform their club that they are playing an exhibition game and provide the game details (opponent, date, time and venue)
This is not an approval process but rather a reporting process. Clubs still have blanket approval to play exhibition games between EODSA clubs within EODSA boundaries. This process will simply ensure that Ontario Soccer policies are being followed and Discipline and Special Incidents are dealt with appropriately.
- One of the clubs will assign a referee
- The teams will fill out the game sheet for exhibition games
- The referee will submit the game sheet to EODSA with caution reports, dismissal reports and special incident reports
- The EODSA will follow up on any discipline required.
